Profile Requirements
Please fill out our online application. Prior to completing your Profile, please be prepared with all documents and photos as listed below.
(If you are missing one-two requirements, please upload a “filler” file in order to move forward until that item is collected/purchased.)
Completed profiles include:
- Contact info (phone, home mailing address, email)
- Bio for website
- Headshot for website
- Clinic info
- Copy of current veterinary license(s)
- All dentistry/anesthesia/surgery CE
Photos of all anesthesia equipment and set up:
- Monitoring with screen on (ECG, BP, Pulse Ox, Temp, +/- CO2)
- Anesthesia machine
- Patient warming systems
- IV fluid pumps
Photos of Digital Dental Radiography:
(Film will not be accepted)
- X-ray generator
- DR Sensor and/or CR Plates
- Software and viewing stations
Photos of dental equipment and set up:
(Please make sure all items are clearly visible, are not stacked on top of each other, the camera is in close range, and instruments are visible in sterilization pouch.)
